The Individuals with Disabilities Education Improvement Act (IDEA)
defines transition as:
A coordinated set of activities for a child with a disability that:
is designed to be within a results-oriented process, that is focused on improving the academic and functional achievement of the child with a disability to facilitate the child’s movement from school to post-school activities, including post-secondary education, vocational education, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation;
is based upon the individual child’s needs, taking into account the child’s strengths, preferences, and interests;
includes instruction, related services, community experiences, the development of employment and other post-school adult living objectives, and, if appropriate, acquisition of daily living skills and functional vocational evaluation.

Meghan has served in the public school system for nearly two decades. She spent six years as an Academic Life Skills teacher at the middle school level and another six years teaching Academic Life Skills/Enclave at the high school level before stepping into the role of transition coordinator. Supporting students through transition has always been her passion.
Patrick Cadigan
Patrick Cadigan
Patrick has been a special educator in the public school system for nearly a decade. He began his career at a public separate day school before transitioning to an Academic Life Skills position at the middle school level. During COVID, he shifted into a resource teacher role and later supported the school’s regional program. Most recently, Patrick took the initiative to move into a transition coordinator position.
